Photography Consent
I understand that my child’s likeness, image, video presence, voice or other reproduction may be used by Cotton Candy Clouds for incidental advertising, website images, media, or other purposes. I further understand that no compensation will be provided for use of my child’s image and that I may not be informed in advance of the specific use of my child’s image. I understand that at any time, I may opt out of this photography consent clause by providing a written statement expressing my desire. I hereby waive all rights of privacy and publicity in relation to the above.
Authorization/ Release of Liability
I have given my consent for my child/children to participate in events being organized by Cotton Candy Clouds. I understand that there are inherent risks involved in any event, and I hereby waive, release, and hold harmless Cotton Candy Clouds from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my child/children’s participation. I release and hold harmless Cotton Candy Clouds and its employees and/or agents from liability in case of accident to myself or my child/children during the events. I understand that my child’s participation in this activity is purely voluntary and that my child is responsible for his/her behavior at all times. My signature below certifies that I, as parent/guardian with legal responsibility for the above participant/s, do consent and agree to indemnify and hold harmless Cotton Candy Clouds from any and all liabilities incident to my minor child’s involvement or participation in the events, even if arising from their negligence, to the fullest extent permitted by law.
Live Animal Waiver
I am aware that participation in interacting with live animals provided by cotton candy clouds may involve physical activities that could potentially lead to sickness, injury or death. I am voluntarily participating in these activities with knowledge of the risks involved, and agree to assume any and all risks of bodily injury, death, property damage, or other loss, whether those risks are known or unknown. I am expressly assuming the risks associated with my participation in this activity.
